An area man is facing 31 felony counts after officials said he defrauded Ohio’s Medicaid program of more than $12 million.
A Butler County grand jury indicted Robert Haley on accusations of corrupt activities, aggravated theft, Medicaid fraud, forgery, and other offenses, according to WCPO.
The Ohio Department of Medicaid referred Haley to the state’s Medicaid Fraud Control Unit, which began investigating her in early 2025.
Investigators discovered substantial fraudulent billing activity associated with QIS LLC, a behavioral-health facility owned and operated in Butler County by Haley, a licensed social worker and independent chemical-dependency counselor.
Between January 2020 and May 2026, Haley was accused of submitting more than 60,000 fraudulent Medicaid claims for treatments that were not given, resulting in over $12 million in erroneous payments.
The alleged fraud includes charging Medicaid for therapeutic behavioral services provided to children in Butler County after-school programs.
Family members of the youngsters said investigators they had no knowledge of the services and that their signatures on consent forms and service plans seemed to be falsified.
